Lawyers' Reporting, Inc. Terms of Service (Faxed/Emailed Transcript Order Form)
| A. | This order form is a binding contract between the above listed company and Lawyers' Reporting, Inc. |
| B. | Additional fees apply to any and all orders other than 10+ Business Days (Standard Delivery) checked in section 3 including additional services checked in section 2. If no delivery is checked in section 3, this order will be 10+ Business Days (Standard Delivery). |
| C. | Cancellation of this order must be in writing and faxed to Lawyers’ Reporting, Inc. at the number above. Upon cancellation, the above company agrees to pay for any and all work completed. |
| D. | The above company acknowledges the opportunity to a written price estimate from Lawyers’ Reporting, Inc. prior to ordering goods and/or services. The above company agrees that Lawyers’ Reporting, Inc.’s charges and rates for goods and/or services are both fair and reasonable. |
| E. | The above company agrees to pay the corresponding invoice(s) for this order within 30 days of the invoice date or C.O.D. including any and all unpaid Lawyers’ Reporting, Inc. invoices. If the corresponding invoice is not paid within 30 days, the above company agrees to pay a compounding interest rate of 1.5% per month on the balance due. |
| F. | The above company agrees to word indexes as part of the total page count for a transcript. |
| G. | The above company agrees that payment for the corresponding invoice for Lawyers’ Reporting, Inc.’s goods and/or services is not contingent upon any third party (including client) reimbursement. |
| H. | The above company agrees to waive any and all statute of limitations on any and all unpaid invoices from Lawyers’ Reporting, Inc. |
| I. | Should any dispute arise, the above company agrees to pay any and all court costs, any and all administration fees and any and all attorney’s fees Lawyers’ Reporting, Inc. incurs as a result of the dispute if Lawyers’ Reporting, Inc. is the prevailing party. The venue for any and all disputes will be in Palm Beach County, Florida. |
| J. | This agreement supersedes any and all prior or contemporaneous oral or written statements, understandings or agreements with respect thereto. No amendment to or modification of this agreement will be binding unless in writing and signed by a duly authorized representative of the parties. |
| K. | These terms of service are also publicly available online at http://www.lawyersreporting.com/services |
| L. | AS AN AUTHORIZED AGENT FOR THE ABOVE COMPANY, I HAVE READ AND AGREE TO ALL THE TERMS OF SERVICE. |
